2025-26 Health Insurance Waiver Form
To waive health insurance coverage for 2025-26, and be included in the count for opt out payment (per your union contract), download and fill out the waiver form and send it and a copy of your health insurance card to Amy Phillips, Central Business Office no later than May 30, 2025.
